New and Improved- Reading Forms

Happy Tuesday Friends! I hope that you had a wonderful weekend! I am sure you are counting down the days until the end of the year! I usually am, but this year I want time to slow down so I have more time with my baby girl!

Since I've been home on maternity leave, I've had some time to update my past products- ones that I made right at the beginning when I didn't really know what I was doing. Now I have so many more backgrounds, fonts and clip art-so I figured it would be good for a little update. So, my Planning and Conference Forms for Reading are {New and Improved}!

Although I am not always the {best} at keeping notes during Reading Groups and Reading Conferences, I always try to stay organized and take notes on my students *Habits* (what they are doing well) and *Goals* (what they need to work on). These are the forms that I made to plan my conferences and groups each week and record all of my notes!

I give each student a Habits and Goals sheet (copied on card stock and laminated) to keep in their Reading Bags (I just bought cheap ones at the Dollar Store), so that when we conference I can just add post-its to remind them of what they are doing well and what they need to work on.

Then, I also record this information in my notebook on my Individual Student Note pages. There's one for the boys and one for the girls!

I also have planning sheets for each week! You can use these to plan out your Reading Conferences and Reading Groups for each week! I made 2 different versions- one that is all one page and then one that is split into two pages!

And here is the notes page for when you are working with your groups!

If you need any additional pages, please let me know and I would be more than {happy} to add them to this little resource!

Also, I updated my Solid Shapes Around the Room and my Place Value Around the Room,  just in case you need some fun activities to use for review at the end of the year!

Happy Teaching,
